The request for a waiting time transfer must be signed by the transplant candidate and submitted to UNOS by the transplant program where the candidate is starting a new listing or transferring time. The transplant program must provide additional information to allow UNOS to verify and complete the transfer process.

However, specific questions relating to a candidate's listing or current status are best addressed by staff members at the candidate's transplant program.
